* Psychcentral.com has a section called "quizzes" and in there is the anxiety screening quiz. There are 22 questions which you rate from Usually to Never. You then hit "submit" and they'll come back with a score. You then compare your score with the range given. They don't give you any suggestions but do give a number of disclaimers so the scores won't be used inappropriately.
* Healthyplace.com is another site with a great deal of information and some anxiety surveys. The problem is that they are difficult to find. You need to look for the section called "psychological tests" on the right. You need to scroll down until you find it.
Answering these questions will give you information about your emotional condition. Remember, the questionnaires are simply to help you make a decision about what you need to do next.
